The QT line comes before any 2.5 mg swallow

The QT line is the first walk. Levitra's US label tells men with congenital long QT, or those on quinidine, procainamide, amiodarone, or sotalol, to avoid vardenafil. Only after that line is clean do I talk about a tablet. This gallery hangs 2.5 mg because it is the smallest film-coated strength and the CYP3A4 cut, not because it is the usual first start. Usual start remains 10 mg about 60 minutes before activity. 2.5 mg is what the label reaches for with ritonavir (one tablet in 72 hours) and with several other strong CYP3A4 inhibitors (once daily, not more). The fuller molecule sits in the vardenafil profile. The class clocks sit in the three-clock comparison.

Walk the tracing before the orange tablet

A first Levitra evening that skips the QT question is not a first walk. It is a skip. In 59 healthy men, 10 mg and a supratherapeutic 80 mg dose moved QTc in the same band as moxifloxacin 400 mg. Fridericia-corrected change versus placebo at one hour was 8 msec after 10 mg and 10 msec after 80 mg. Heart rate rose about 5-6 beats per minute.

A later study in 44 volunteers found a 5 msec QTcF change after 10 mg, 4 msec after gatifloxacin 400 mg, and 9 msec when both were given. The label calls the clinical impact unknown and still names the avoid list: congenital long QT, Class IA, Class III. I will not pretend 2.5 mg deletes that paragraph.

If the tracing is unclean or the antiarrhythmic is on the avoid list, stop shopping milligrams. Ask about sildenafil 25-100 mg or daily tadalafil 5 mg instead, with the same nitrate hold. Vardenafil is the clock with the extra walk. That is its job on this wall, not a branding slight.

Ritonavir turns 2.5 mg into a 72-hour tablet

Ritonavir 600 mg twice daily with vardenafil 5 mg raised vardenafil AUC 49-fold and Cmax 13-fold and stretched half-life to about 26 hours. That is why the labelled answer is a single 2.5 mg tablet in 72 hours, not a daily 2.5 mg strip and not a 'small 10 mg.' Indinavir raised AUC 16-fold; the cap there is 2.5 mg in 24 hours.

If the antiviral list changed last month, the vardenafil sentence has to be rewritten. Boosters are not interchangeable folklore. Bring the current bottles. A 2.5 mg tablet that was correct on the old regimen can be wrong on the new one, in either direction.

Long-term safety with protease inhibitors was already called out as limited on the old Levitra language. That is another reason this is a prescriber walk, not a cart walk. I will not bless a 72-hour home calendar from here.

Sixty minutes, then a short evening

Fasted Tmax is 30 minutes to 2 hours, median 60. That is the same midpoint as sildenafil and a different leftover. Half-life is about 4-5 hours. Plan the evening you are in. Do not plan the next afternoon.

Absolute bioavailability is about 15 percent. Most of an oral dose leaves as metabolites in feces (about 91-95 percent). Little appears in urine. Protein binding is high, in the mid-90s. Those numbers explain a small milligram for a real effect. They do not make 2.5 mg equal to 10 mg on a first healthy start.

Stimulation still has to fire. Vardenafil does not start desire. A first walk that is only a swallow and a wait will look like a failed cut dose. Give the hour, then give the evening a real chance, then judge.

A high-fat plate can flatten the peak

Two food studies showed high-fat meals cutting vardenafil Cmax by 18 to 50 percent. That is a wider bruise than sildenafil's mean 29 percent Cmax cut, and it is still a rate-and-peak story. A first 2.5 mg tablet after a cream sauce is a weak exam of a cut dose. A first 10 mg tablet after the same sauce is a weak exam of a usual start.

You do not need a fast. You need a first two evenings that are not a food test. A lighter plate, or a gap after the rich one, is the practical advice. Daily tadalafil 5 mg is the clock that stops negotiating dinner. Vardenafil is not that clock.

Alcohol is a separate modifier. It can make a flush or a faint feeling easier to earn. It does not cancel the QT walk or the nitrate ban. I would rather a first walk be a clear head and a lighter plate than a 'relaxed' heavy night that proves nothing.

Film-coated Levitra comes as 2.5, 5, 10, and 20 mg. The recommended start for most men is 10 mg about 60 minutes before, then 5 or 20 on response. Once per day is the cap. Age 65 and over, or Child-Pugh B, starts at 5 mg. Child-Pugh C was not studied. 2.5 mg is not the default 'gentle' first night for a healthy man with an empty list.

Ritonavir: one 2.5 mg tablet in 72 hours. Indinavir, saquinavir, atazanavir, clarithromycin, ketoconazole 400 mg daily, itraconazole 400 mg daily: do not exceed 2.5 mg once daily. Ketoconazole 200 mg or erythromycin: 5 mg cap in 24 hours. Those sentences are why a 2.5 mg box in the bag should make you read the rest of the bottles, not shrug that the pharmacy was timid.

If the box is 2.5 mg and the list has none of those drugs, ask why. A lowest-start plan with a stable alpha-blocker is one honest answer. A mix-up is another. Do not 'correct' it to 10 mg from a leftover blister. Do not cut a 5 mg tablet and call that 2.5 mg science.

Class IA and III stay off this card

The walk, in one list

Quinidine and procainamide are the Class IA names the label writes. Amiodarone and sotalol are the Class III names. Congenital long QT is the tracing name. Those are avoids, not 'use 2.5 mg and be careful.' Additive QT with another QT-active drug was shown with gatifloxacin. The meaning is called unknown. The avoid list is not.

Flecainide is Class IC. It is not the named pair. A new arrhythmia drug of any class still belongs in the same visit as a first PDE5 tablet. I will not sort that from a letter. I will say: do not start Levitra from a friend's leftover because someone called it stronger per milligram.

In vitro, vardenafil is a tight PDE5 binder. That tightness is why the milligram numbers look small next to sildenafil 50 mg. Tightness is not a clinical trophy. A man who did well on 10 mg vardenafil and poorly on a fed 50 mg sildenafil may be looking at food and titration, not at a potency ranking from a bench assay.

Alpha-blockers and the lowest start

If an alpha-blocker is already stable, a PDE5 inhibitor starts at the lowest recommended dose. On the Levitra card that lowest film-coated tablet is 2.5 mg. That is an honest reason to see 2.5 mg in the bag without a protease inhibitor on the list. Standing dizziness is the watch, especially the first evenings.

Do not add vardenafil the same week the alpha-blocker is being titrated. Stability first, then the lowest start. If both drugs are being used as prostate hypotensives, stop and have the indication fight in clinic, not in a bathroom. Tadalafil 5 mg daily has its own BPH-stack warning with alpha-blockers. Vardenafil is not a workaround for that fight.

Other antihypertensives still add a little extra drop. The nifedipine study with vardenafil 20 mg showed about 6/5 mmHg extra supine reduction. That is a counseling number, not a reason to skip the QT walk. First evening: stand slowly, skip the hot-bath experiment, do not stack a large drink.

After the walk, one short evening

If QT is clean and the list is empty, 10 mg about an hour before, lighter plate, real stimulation, one tablet that day: that is a usual first walk. If the list forced 2.5 mg, keep the same hour and plate rules and do not 'make up' the milligrams. If QT is unclean, step off this card.

Compare leftovers in the three-clock review if the real wish is a daily 5 mg tadalafil plateau or a sildenafil evening you already understand. The vardenafil profile keeps the molecule in one place. This dispatch only exists to slow the first swallow.

Nitrates stay banned. In the 20 mg plus 0.4 mg nitroglycerin study, extra blood-pressure and heart-rate effects were gone by 24 hours in healthy subjects. That is a residual-clock fact. It is not permission. Say vardenafil and the hour if chest pain starts. Then let a team work.

My uncle has long QT. I have never been traced. Can I still try 2.5 mg because it is the small tablet?

A family story of long QT is exactly why you do not start from a leftover. Congenital long QT is a named avoid on the vardenafil label. 2.5 mg does not punch a hole in that sentence. Ask for your own ECG and a family history taken by someone who will write it down, before anyone hands you an orange tablet.

If your tracing is clean and the list is empty, the usual start is still 10 mg, not 2.5 mg. The small tablet is a cut for interactions or a lowest start with a stable alpha-blocker. It is not a hereditary-risk discount.

If the family story stays muddy, sildenafil or tadalafil may be the cleaner clocks, still with a nitrate hold. That choice is not mine from a mailbox. It is a visit with the tracing on the desk. Do not borrow his leftover to 'see.'

He is on amiodarone. A clinic nurse said Levitra is fine at 2.5 mg. Is that the label?

No. Amiodarone is a Class III name the label writes in the avoid line next to sotalol. Patients on those drugs should not use vardenafil. A 2.5 mg tablet is not a workaround. The thorough QT work put 10 mg in the same QTc band as moxifloxacin. Additive QT with another QT-active drug was shown later. The clinical meaning is called unknown. The avoid list is not optional.

Ask the clinic which PDE5 inhibitor they meant. Sildenafil 25-100 mg or daily tadalafil 5 mg may still be discussable after the nitrate question and the rest of the list. Vardenafil should not be the one they 'make small' to keep amiodarone comfortable.

Do not stop amiodarone to make space for Levitra. Rhythm drugs are not a lifestyle layer. The conversation is which erection tablet, if any, fits the rhythm plan that already exists.

I take ritonavir and was told 2.5 mg. Can I take that every Saturday, or is the 72-hour gap from the last Saturday?

The gap is 72 hours from the last vardenafil swallow, not 'one Saturday a week' as a slogan. Saturday to Saturday is 168 hours, so a true weekly swallow would clear that gap. Saturday plus a Tuesday 'boost' would not. The labelled sentence is a single 2.5 mg tablet in a 72-hour period with ritonavir.

Ritonavir raised vardenafil AUC about 49-fold in the interaction study and stretched half-life to about 26 hours. That is why the calendar is hard. Do not add a 5 mg tablet because Saturday felt thin. Do not borrow a 10 mg from a friend midweek.

If Saturday weekly is the real rhythm of your life, say so, and ask whether that 2.5 mg weekly plan is what they meant. If the booster changes, the sentence changes. Bring the current antiviral list every time someone rewrites the ED tablet.

First 10 mg after a late dinner did nothing. Should we jump to 20 mg or first retry 2.5 mg on an empty stomach?

Retry the start you were given, with a lighter plate and a full hour, before you jump. High-fat meals cut vardenafil Cmax by 18 to 50 percent in the food studies. A late dinner is a common way to fail a 10 mg exam. Jumping to 20 mg after one fed evening is how people collect a headache and still do not know the drug.

2.5 mg on an empty stomach is not the usual retry if 10 mg was the prescribed start and the list is empty. That is a weaker tablet plus a better plate, which mixes two variables. Keep 10 mg. Clear the plate. Give stimulation. Try two or three evenings.

If those fair evenings are still thin, 20 mg is the labelled step up, once daily maximum. If 10 mg works and the flush is too much, 5 mg is the step down. QT and the list still sit under every step. The vardenafil profile keeps that ladder in one place.

I use tamsulosin. The box says 2.5 mg. Is that the alpha-blocker start, and can I take it the same morning as tamsulosin?

Dr. Elin Dahl answers

Lowest recommended start with a stable alpha-blocker is a labelled caution across this class. On Levitra that lowest film-coated tablet is 2.5 mg. So the box can be right even if you have no ritonavir. The watch is standing dizziness, especially the first evenings, not a new stopwatch.

I want the alpha-blocker dose stable first. Starting both the same week, or raising tamsulosin the same week as the first vardenafil, is how people earn a faint. Same-morning swallows are less important than stability and a slow stand. Some clinicians separate the hours. That is local habit. Stability is the rule.

If tamsulosin and vardenafil are both being used as prostate hypotensives, that is a different fight. Bring both indications to the visit. Do not add a third hypotensive 'for the stream' because two were not enough.

Noor asks Q06

If he gets chest pain four hours after 2.5 mg, is the 24-hour NTG study a green light for the spray?

Dr. Elin Dahl answers

No. Four hours is still inside the window where the 20 mg plus nitroglycerin study saw extra blood-pressure and heart-rate effects (those extra effects were seen at 1 and 4 hours for blood pressure, and at 1, 4, and 8 hours for heart rate). They were not seen at 24 hours. The contraindication does not wait for that 24-hour mark.

Call emergency services. Say vardenafil and the hour and the milligram. Do not use the spray. Do not wait to 'reach 24 hours' while pain continues. The 24-hour sentence is a residual-clock fact for teams. It is not a home permission.

After the night, nitrates and vardenafil cannot share a plan. That is true at 2.5 mg and at 20 mg. The three-clock review hangs the same ban on all three cards. The leftover hours differ. The ban does not.
